Spacious semiprivate and private rooms

Featuring modern decor, private bathroom, in-room telephone, and a flat-screen TV with cable service.

Skilled rehabilitation services

Progressive rehabilitative therapies are specifically designed to bring Health Center residents back to good health and into their homes.  Our restorative nursing program helps residents build up strength and endurance in ambulation, range of motion, positioning and alignment, and contracture alignment, even after rehabilitative therapy has been discontinued. This continuum of care offers Health Center residents the opportunity to move to Baywood Court’s Assisted or Independent Living accommodations when ready.

24-hour skilled nursing care

Licensed nurses provide skilled, individualized care and treatments that include diabetic management, Intravenous (IV) antibiotic therapy, pain management, and enteral feeding. Post-surgical care, advanced wound care, hemodialysis care, cardiac care, stroke rehab, and post-chemo and radiation recovery are also provided as needed. 

 

Physical therapy

Physical therapy focuses on identification and rehabilitation of individuals with physical weakness, deconditioning, balance impairment and pain, as well as training techniques and body mechanics to reduce risk of future injury. Our physical therapy team’s goal is to develop a program that is based on individualized needs.

Occupational therapy

Our occupational therapists work with residents in adapting to their social and physical environment by targeting individualized tasks essential for daily living. An occupational program might include working on functional independence for things such as cooking, eating, bathing, toileting, dressing and grooming. Our therapists also work with residents in increasing safety awareness for when residents return home.

Speech and language pathology

Our speech and language therapists work with residents affected by stroke, brain injury, Parkinson’s, dementia or those suffering from hearing impairment, respiratory illnesses and other complex medical conditions such as gastronomy tube placement. The speech therapy program is designed to improve voice, increase comprehension and verbal expression, improve chewing and swallowing and maximize cognitive skills.

 

Long-term care and respite programs

In addition, we provide long-term care programs and respite programs that allow full-time caregivers guilt-free personal time at home. Family members can be reassured their loved one is getting the personalized care and service that they are used to. 

Hospice program

For those entering the extremely difficult end-of-life phase, our hospice program helps to provide comfort and support to residents and their families.

Designed to keep you moving

The therapists at Baywood Court Health Center work closely with each resident and family to understand their goals and achieve their highest functional potential as quickly and efficiently as possible. We provide state-of-the-art equipment and highly trained staff with expertise in orthopedic rehab, cardiopulmonary rehab, stroke/neurologic conditions, pain management, manual therapy techniques, communication/cognition, and swallowing impairments. Progress towards goals is assessed weekly in order to monitor the treatment plan to ensure clinical needs are met and/or exceeded.  Once rehabilitation goals have been achieved, discharge planning occurs, which includes home assessments and in-depth caregiver training to support a resident’s continued success in their own environment.
