Baywood Court is currently recruiting Social Worker/Discharge Planner for our soon to open Health Center (Skilled Nursing Facility). The general job summary is as follows;

· The Social Worker/Discharge Planner is responsible for assisting staff, residents and resident families to understand and cope with residents’ personal, emotional, and related health and environmental problems.

· All job functions are performed with customer service and hospitality as the overall goal to both our residents and co-workers alike.
